This toolkit helps caregivers, families, and young people make shared choices about treatment. It can help kids and teens have a voice in their care. It can also guide family members, caregivers, and foster parents to help their children get the right care and have great health outcomes. The Shared Decision Making Toolkit was crafted to be printable and taken to one’s medical and/or psychiatric appointment(s). It can be helpful to work through each section with one’s care team in order to develop a suitable care plan.
